Patients First: Sub-Region Planning Consultation-Western Champlain

Please join us at the Sub-Region Planning Consultation in Western Champlain: The Champlain LHIN is undertaking several consultations to determine sub-region planning and health service delivery areas within the larger Champlain region. The new planning approach is part of Ontario’s new Patients First: Action Plan for Health Care for health system transformation.We invite you to register a maximum of two representatives from your organization.We encourage you to  invite a patient/client or caregiver to participate in this session.

Materials for the session will be available in French and English. Please indicate your language preference for small group discussions on the registration form.

Register for the face to face consultation session before September 5, 2016. Should you be unable to attend this session in person, an online session in French and English will be available during the week of September 12th, 2016. Details will follow.
